You may submit an application via our online system. If you wish to submit a paper application, please utilize the links below to obtain documents for doing such. Otherwise, if you wish to submit an application online, you must register with the system. Please start a new application, which includes creating a user name and password for identification.

Recommendation Forms (Print out 2 copies each) from current or previous employer or other responsible individuals (non-family members)
2 passport size photographs
Official transcripts or official equivalent documentation sent directly to ACAOM from colleges previously attended.
Personal essay (topic listed on the application)
Copy of valid personal identification
A non-refundable application fee of $50 payable to ACAOM by the deadline. Applicants submitting after the deadline must enclose $100.

Official transcripts or official equivalent documentation sent directly to ACAOM from colleges previously attended. If transcript is from an international institution, then a credential evaluation is required. Students can seek an evaluator such as Global Credential Evaluators .
Personal essay (topic listed on the application)
Copy of valid personal identification
A non-refundable application fee of $150 payable to ACAOM by the deadline. Applicants submitting after the deadline must enclose $200.
Copy of TOEFL score of at least 500 on paper-based or 173 on computer-based TOEFL for applicants from non-English speaking countries.
Proof of financial responsibility, either in the form of a bank statement or affidavit of support showing a guaranteed support of at least U.S. $22,370.
